To the Editor.— Turkel and associates1 postulate that gross bilirubin staining of newborn infants' brains, as observed at autopsy, is only incidental and does not correlate with specific microscopic changes as described with "true kernicterus": microscopic study of brain sections from 32 infants in whom the typical gross bilirubin staining of kernicterus was found at autopsy, showed "microscopic changes in the target areas of classic kernicterus" in only three patients while "diffuse spongy change" was more common.
