Long-term use of olanzapine may adversely affect oral health by disrupting bone homeostasis and increasing the risk of alveolar resorption and structural bone damage. This preclinical experimental investigation aimed to elucidate the role of olanzapine in modulating bone homeostasis during orthodontic tooth movement, with a specific em phasis on the expression of Dentin Matrix Protein 1 (DMP1), a pivotal marker of osteocyte activity and skeletal remod eling. Thirty male Wistar rats were randomly allocated into experimental groups receiving olanzapine (2 mg/kg/day, orally), orthodontic force, or a combination thereof, along side a control group. Orthodontic force was applied for five weeks using a closed-coil superelastic spring. Analyses were conducted on days 21 and 56, including histomor phometric evaluation of alveolar bone and quantitative gene expression analysis via RT-PCR for Dmp1, with GAP DH as the endogenous control. Short-term administration of olanzapine led to a statistically significant downregula tion of Dmp1 expression on day 21 compared to controls (P = 0.0003; P = 0.0228). On day 56, Dmp1 expression remained significantly suppressed in the group subjected to combined olanzapine treatment and orthodontic force (P = 0.0021; P = 0.0432), indicating a sustained interfer ence of olanzapine with bone remodeling pathways under mechanical loading conditions. The data suggest that olanzap ine impairs osteocyte maturation and function by attenuating Dmp1 expression, thereby potentially disrupting alveolar bone remodeling during orthodontic tooth movement. These findings underscore the need for careful skeletal monitoring in patients undergoing long-term olanzapine therapy, particularly those ex posed to orthodontic or biomechanical interventions.
