Comparative Effectiveness of Ajamodadi Vataka and Simhanada Guggulu in the Management of Amavata (Rheumatoid Arthritis): An Open Label Double Arm Prospective Clinical Study

Background: Amavata, the Ayurvedic correlate of Rheumatoid Arthritis (RA), is a chronic inflammatory joint disorder mediated by the combined vitiation of Ama and Vata Dosha. Both Ajamodadi Vataka and Simhanada Guggulu are classical Ayurvedic formulations prescribed in Amavata, but their comparative efficacy needs systematic evaluation. Objective: To compare the therapeutic effectiveness of Ajamodadi Vataka and Simhanada Guggulu in subjects diagnosed with Amavata using objective inflammatory markers (ESR, CRP, RA Factor) and the Disease Activity Score-28 (DAS28). Methods: An open label, double arm, prospective clinical study was conducted on 30 subjects in each group (n = 30 per arm, total n = 60). Group A received Simhanada Guggulu 1 g thrice daily with warm water; Group B received Ajamodadi Vataka granules 4 g thrice daily with warm water for a defined treatment duration. Subjects were assessed at baseline and post-intervention for ESR (mm/hr), CRP (mg/L), RA Factor (IU/mL), DAS28 score, swollen joint count (SJC), tender joint count (TJC), and patient global health assessment. Statistical analysis used the paired t-test for within-group comparisons and the independent samples t-test for between-group comparisons. Results: Both groups demonstrated statistically significant (p < 0.001) improvements in DAS28 and ESR. Simhanada Guggulu produced a mean DAS28 reduction of 1.00 ± 0.40 (17.9%), while Ajamodadi Vataka produced 1.02 ± 0.29 (19.8%). ESR reduction was 7.80 ± 3.39 mm/hr and 7.30 ± 3.55 mm/hr respectively. Between-group comparison revealed no statistically significant difference in any parameter (p > 0.05). Conclusion: Both Ajamodadi Vataka and Simhanada Guggulu are equally effective and clinically beneficial in the management of Amavata, with comparable reductions in disease activity, inflammation, and joint pathology.
