Hernie de Claudius Amyand : à propos de 3 cas à l'Hôpital National de Niamey.

Introduction : La hernie de Claudius Amyand est définie comme une hernie de la paroi abdominale dont le sac contient l'appendice vermiculaire normal ou pathologique. Le diagnostic est souvent peropératoire. L’objectif de cette étude était de décrire les circonstances diagnostiques et les modalités thérapeutiques de trois cas de hernies de Claudius Amyand. Observations : Nous rapportons trois patients de sexe masculin âgés de 50, 38 et 48 ans, admis aux urgences pour tuméfaction inguino-scrotale droite douloureuse et non réductible, avec un syndrome occlusif aigu. Le diagnostic d’une hernie inguino-scrotale droite étranglée a été retenu, redressé en per-opératoire en hernie de Claudius Amyand. La prise en charge a consisté en une appendicectomie et une herniorraphie selon Bassini (2 cas) et une résection iléo-colique (1 cas). Les suites opératoires étaient simples. Conclusion : La hernie de Claudius Amyand est rare ; dans notre contexte le diagnostic se fait fréquemment en peropératoire. Observation: We report the case of three male patients aged 50, 38 and 48 respectively, with no notable personal or family pathological history, admitted to the emergency department with painful, non-reducible right inguino-scrotal swelling. Clinical examination revealed an acute occlusive syndrome in all three patients; the rest of the clinical examination was unremarkable. Biological tests were normal in one patient and abnormal in the other two. No imaging examinations were performed. The diagnosis of a strangulated right inguino-scrotal hernia was retained for each of the 3 patients. Intraoperatively, the diagnosis was changed to Claudius Amynad hernia. Surgical management consisted of appendectomy and Bassini herniorrhaphy in two cases, and ileocolic resection in one patient. Postoperative management was straightforward, and all patients had a favourable outcome. Conclusion: Claudius Amyand hernia (CAH) is rare. An abdominal CT scan is used to make the diagnosis. In our context, the diagnosis is frequently made intraoperatively.
