Intérêt de l’électrocardiogramme dans le diagnostic de Situs inversus totalis en milieu rural : À propos d’un cas et revue de la littérature.

Introduction : Le situs inversus complet est une malformation congénitale rare. Il est asymptomatique est compatible avec une vie normale. Le diagnostic de situs inversus totalis est souvent posé fortuitement à l’âge adulte au décours d’un examen paraclinique. L’objectif de ce travail était de rapporter un cas de situs inversus totalis de découvert à l’occasion d’un électrocardiogramme (ECG) du bilan d’une hypertension artérielle (HTA) chez un patient de 59 ans. Observation : Un patient de 59 ans, vivant en milieu rural, hypertendu connu, a consulté notre service pour son suivi médical. L’examen physique notait une pression artérielle élevée à 191/110mmHg, un choc de pointe au 5ème espace intercostal droit. L’électrocardiogramme objectivait des signes évocateurs d’une dextrocardie : inversion de l’onde P en I, aVL et aVR ; onde S dominante en I, et de V1 à V6 avec une progression inversée de l’onde R dans les dérivations précordiales ; un faible voltage de l’axe QRS de V4 à V6 ; un axe du QRS extrême, les ondes T aplaties de V4 à V6 et en aVR et des ondes T inversées dans les dérivations I et aVL. La radiographie thoracique et l’échographie abdomino-pelvienne ont permis de confirmer le diagnostic d’un Situs Inversus Totalis. La nicardipine 20 mg/12 heures avait permis de normaliser la pression artérielle. Le patient fut informé de son état anatomique et avait reçu des conseils sur la nécessité d’informer le médecin traitant à chaque consultation afin d’éviter les erreurs diagnostiques et thérapeutiques. Conclusion : A travers cette observation nous rappelons l’intérêt de l’électrocardiogramme en tant que moyen de diagnostic simple et non invasive de la dextrocardie et d’autres anomalies cardiaques dans les centres à ressources limitées.
