ADDRESSING THE LACK OF PERIMENOPAUSE CONVERSATIONS BETWEEN WOMEN AND HEALTHCARE PROFESSIONALS

Abstract
Objectives
Perimenopause can adversely impact sexual health among women and, conversely, negatively affect overall well-being and quality of life (QOL). Yet, it is often dismissed by healthcare professionals (HCPs). This study aimed to understand the sexual health needs and concerns of women during the perimenopausal transition, as well as their perceptions of healthcare experiences related to perimenopause and sexual health.
Methods
This qualitative study used semi-structured interviews with 25 women ranging in age from 38 to 54 years old who self-reported being in perimenopause. Additionally, the Menopause Rating Scale (MRI) (Heinemann, 2004) was used with a cutoff score of 5 points as an inclusion criterion. Thematic data analysis with the reduction of data into themes was applied to identify recurring patterns, themes, and meanings within the data (Creswell & Creswell, 2017).
Results
We identified several major themes, capturing the multifaceted experiences of participants encompassing interactions with HCPs concerning issues related to perimenopause and sexual health. These themes reflect salient features of the participants’ narratives, revealing the complexities and nuances inherent in their encounters with their providers. The identified themes are as follows: (1) insufficient sexual health information and preventative education; (2) lack of open discussion about sexual health; (3) need for enhanced support and treatment options; (4) feeling dismissed by a healthcare professional; and (5) sexual pleasure not addressed by the healthcare community.
Conclusions
Findings demonstrate women in perimenopause often experience unmet sexual health needs during healthcare visits, particularly in adequately addressing changes in sexual function and satisfaction. A lack of in-depth discussions on sexual health revealed a critical gap in comprehensive care, highlighting a need for enhanced medical education and training to better prepare HCPs for approaching sexual health conversations. To better support women during perimenopause, HCPs should adopt a more empathetic patient-centered approach that validates women’s experiences, recognizing the significant impact of perimenopause symptoms on sexual health and overall well-being. A paradigm shift in healthcare is necessary, where sexual pleasure is acknowledged, discussed, and prioritized as an integral component of holistic, affirmative, and patient-centered sexual health care.
