Comprehensive Ayurvedic Management of Gridhrasi - A Case Study

Introduction: Gridhrasi, described under Vata-Vyadhis in Ayurveda, clinically resembles sciatica characterized by radiating pain from the lumbar region to the lower limb. The condition arises primarily due to vitiation of Vata Dosha, sometimes associated with Kapha Dosha (Vata-Kaphaja Gridhrasi). It commonly affects individuals in their productive years and significantly hampers mobility and daily activities. This case report evaluates the effect of a Panchakarma-based Ayurvedic management in acute-on-chronic Gridhrasi.
Methods: A clinically diagnosed case of acute-on-chronic Gridhrasi was managed with a structured Ayurvedic treatment protocol comprising Sarvanga Abhyanga, Swedana, Kati Basti, and Yoga Basti administered as per classical guidelines. Treatment aimed at pacifying aggravated Vata, reducing pain and stiffness, and restoring functional mobility. Clinical assessment was based on pain intensity, range of motion, stiffness, and gait before and after intervention.
Results: Post-treatment, the patient demonstrated significant reduction in pain and stiffness with marked improvement in lumbar mobility and gait. Functional capacity improved considerably, enabling the patient to resume routine activities without discomfort. No adverse effects were reported.
Discussion: As Gridhrasi is predominantly a Vata-Pradhana Vyadhi, therapies focusing on Vata Shamana and Anulomana are essential. The combined effect of external therapies and Yoga Basti effectively addressed the underlying Dosha Dushti and improved functional outcomes. This case highlights the therapeutic potential of a comprehensive Ayurvedic approach in managing Gridhrasi.
