Effect Of Snigdha Agni Karma In Gridhrasi- A Review

Gridhrasi is one of the most common Vatavyadhi described in Ayurveda, characterized by radiating pain from the lumbar region to the lower limb along with stiffness, numbness, pricking sensation, and restriction of movement. In contemporary medicine, it can be correlated with Sciatica, which significantly affects quality of life and functional ability. Ayurveda emphasizes various therapeutic modalities for the management of Gridhrasi, among which Agni karma holds an important place due to its analgesic, anti-inflammatory, and Vata-Kapha shamaka properties. Snigdha Agni Karma, a modified form of Agnikarma using unctuous substances, has gained attention for providing deeper therapeutic action with reduced tissue damage and enhanced healing. . The review highlights the probable mode of action of Snigdha Agni Karma through improvement of local circulation, reduction of muscle spasm, alleviation of pain, pacification of aggravated Vata, and stimulation of tissue metabolism. The thermal and unctuous properties together help in relieving stiffness and improving mobility with minimal complications. Modern treatments often relieve symptoms but may lead to recurrence and side effects. This situation highlights the need for safe and effective alternative therapies. Snigdha Agni Karma, a modified version of Agnikarma that uses unctuous media with therapeutic heat, has emerged as a promising option for managing Gridhrasi. This critical review aims to assess the impact of Snigdha Agni Karma on Gridhrasi from both Ayurvedic and modern scientific viewpoints. The review looked at classical Ayurvedic texts like Charaka Samhita, Sushruta Samhita, Ashtanga Hridaya, and commentaries by Acharya Dalhana to understand the principles, indications, mechanisms, and therapeutic uses of Agnikarma. According to Ayurveda, Agnikarma reduces aggravated Vata and Kapha Dosha, clears Srotorodha, boosts local circulation, and eases Vedana (pain). The Snigdha aspect provides extra Vatahara and Brimhana effects, lessening dryness and stiffness linked to Gridhrasi. From a modern standpoint, therapeutic heat may work through widening blood vessels, relaxing muscles, enhancing tissue metabolism, and adjusting pain pathways. This review shows that Snigdha Agni Karma is an effective, minimally invasive, affordable, and fast-acting para-surgical method for managing pain in Gridhrasi. It can decrease pain, stiffness, tenderness, and functional limitations while enhancing quality of life. The therapy also illustrates the scientific relevance of traditional Ayurvedic Para surgical methods in today's pain management. Further clinical and evidence-based studies are essential to confirm its therapeutic effectiveness and standardize treatment protocols.
