Clinical Evaluation of Ayurvedic Treatment in Chronic Lichenified Eczema (Vicharchika): A Case Report

Introduction: Chronic Lichenified Eczema, also known as Lichen Simplex Chronicus, is a chronic inflammatory skin disorder characterized by persistent itching, lichenification, hyperpigmentation, dryness, and scaling resulting from a continuous itch-scratch cycle. The condition significantly impairs quality of life by affecting sleep, daily activities, and psychological well-being. In Ayurveda, the clinical features of Chronic Lichenified Eczema closely resemble Vicharchika, a subtype of Kshudra Kushta. Vicharchika is primarily associated with vitiation of Kapha, Pitta, and Vata Doshas involving Twak, Rakta, Mamsa, and Lasika.
Aim and Objective: To evaluate the efficacy of an Ayurvedic treatment protocol in the management of Chronic Lichenified Eczema (Vicharchika).
Materials and Methods: This case study was conducted in the Outpatient Department of Dravyaguna, Government Ayurveda College and Hospital, Pratap Nagar, Jaipur, Rajasthan. A 38-year-old female patient presented with severe itching, hyperpigmented lichenified plaques, dryness, scaling, disturbed sleep, and constipation. Clinical assessment was performed using the Dermatology Life Quality Index (DLQI), Eczema Area and Severity Index (EASI), and Visual Analog Scale (VAS) for pruritus. The patient received Ayurvedic treatment comprising Khadirarishta, Mahamanjisthadi Kwatha, Haridrakhanda, Arogyavardhini Vati, Giloya Satva, Gandhaka Rasayana, Avipattikara Churna, and external applications along with dietary modifications.
Results: Significant clinical improvement was observed following treatment. DLQI improved from 22 to 8 and further to 3 during follow-up. EASI score reduced from 18 to 8 and subsequently to 2, while VAS score for pruritus decreased from 9 to 4 and finally to 1. Marked reduction in itching, lichenification, dryness, scaling, and hyperpigmentation was observed without recurrence.
