Early results of distal metatarsal osteotomy with Kwire fixation in the treatment of tailor’s bunion

Abstract Objective: Tailor’s bunion deformity is a lateral side bone and soft tissue prominence of the fifth metatarsal bone. The aim of our study was to assess the clinical and radiographic results of distal metatarsal osteotomies in patients with tailor s bunion deformity. \r\nMethods: This study included 24 feet of 14 patients with tailor’s bunion who were treated with distal metatarsal osteotomy of the 5th metatarsal between 2006 and 2009. The mean follow-up time was 24.45 (range: 12 to 47) months. Patients were evaluated clinically and radiographically, using the American Orthopaedic Foot and Ankle Society (AOFAS) scoring system with weight-bearing anteroposterior and lateral foot radiographs. \r\nResults: Average AOFAS scores of the patients were 64.83 preoperatively and 91.62 at the final follow-up. Three patients had complications; avascular necrosis, delayed union, and superficial wound infection. \r\nConclusion: Distal metatarsal osteotomy is a safe and easy treatment option for the painful tailor’s bunion deformity and provides patient satisfaction rate of up to 96%. Özet Amaç: Terzi bunyonu deformitesi beşinci metatarsın lateral kenarının kemik ve yumuşak doku çıkıntısıdır. Çalışmamızın amacı terzi bunyonu deformitesi olan hastalara uygulanan distal metatarsal osteotomilerin klinik ve radyolojik sonuçlarını değerlendirmekti. \r\nÇalışma planı: 2006-2009 yılları arasında terzi bunyonu deformitesi nedeniyle beşinci metatarsa distal metatarsal osteotomi uygulanan 22 hastadan 14 hasta (24 ayak) çalışmaya alındı. Ortalama takip süresi 24.45 (dağılım: 12-47) ay idi. Hastalar Amerikan Ayak ve Ayak Bileği Ortopedi Cemiyeti (American Orthopaedic Foot and Ankle Society, AOFAS) skorlama sistemine göre klinik ve radyolojik açıdan değerlendirildiler. \r\nBulgular: Ameliyat öncesi ortalama küçük parmak deformiteleri AOFAS skoru 64.83 olup son takipte 91.62 idi. Bir hastada avasküler nekroz, bir hastada kaynama gecikmesi ve diğer bir hastada ise yüzeysel yara yeri enfeksiyonu olmak üzere, üç hastada komplikasyon görüldü. \r\nÇıkarımlar: Distal metatarsal osteotomi basit, güvenli ve %96’ya varan hasta memnuniyeti ile ağrılı terzi bunyonu deformitesi tedavisinde en uygun yöntemlerden biridir.
