Nefropatia da proteinuria di Bence-Jones

La proteinuria di Bence-Jones consiste nella presenza di catene leggere immunoglobuliniche monoclonali nelle urine. Le catene leggere normalmente vengono eliminate a livello renale ma, quando vengono prodotte in quantità eccessiva, determinano lesioni istologiche e funzionali a livello di tubuli, glomeruli e vasi sia per azione diretta, sia per rilascio di enzimi lisosomiali intracellulari, sia determinando l’ostruzione dei tubuli renali. Trattiamo di questi danni renali in chiave morfologica e funzionale. La proteinuria di Bence-Jones può essere evidenziata attraverso elettroforesi, immunoelettroforesi, immunofissazione elettroforetica, elettroforesi su gel di poliacrilammide con sodio-dodecil-solfato, elettroforesi bidimensionale ed elettroforesi capillare.
