Fistula Karotid-Kavernosa Spontan Bilateral

Pendahuluan : Fistula karotid-kavernosa (carotid cavernous fistule/CCF) adalah adanya komunikasi abnormal antara arterial karotis dengan sinus kavernosus. CCF dibagi menjadi high-flow dan low-flow, spontaneous dan trauma, serta direk dan indirek. Penatalaksanaan konservatif efektif pada pasien CCF indirek, spontan, dan low flow.
Tujuan : Untuk melaporkan kasus CCF bilateral yang indirek, spontan, dan low flow
Presentasi Kasus : Perempuan, 55 tahun datang ke poliklinik Neurooftalmologi pada 17 April 2018 dengan oftalmoplegi, protusio, penurunan penglihatan, dan kemerahan kedua mata sejak 2 minggu terakhir tanpa riwayat trauma. Terdapat edem palpebra dan kemosis bilateral. VODS 6/9. Terdapat esotropia OD dengan oftalmoparesis ke superolateral and inferior. TIO OS 26 mmHg dengan oftalmoplegia. GDP 300 mg/dl, kolesterol 239 mg/dl, dan trigliserida 291 mg/dl. CT angiografi menunjukkan dilatasi vena oftalmika superior bilateral dan terdapat hubungan antara arteri karotis interna dan sinus kavernous. Pasien didiagnosis CCF spontan bilateral, diterapi dengan kompresi manual arteri karotis interna, timolol 2x sehari pada mata kiri, dan metformin 3x500mg.
Hasil : Setelah 2-3 bulan terdapat perbaikan yaitu VODS 6/6 dan hilangnya edem palpebra serta kemosis. Terdapat juga perbaikan gerak ocular, hanya tersisa abduksi pada mata kiri. GDP: 109mg/dl, kolesterol: 187mg/dl, dan trigliserida: 193mg/dl.
Kesimpulan : CCF indirek sering terjadi spontan dan dikaitkan dengan penyakit sistemik. CCF indirek ini umumnya dapat sembuh dengan penatalaksanaan konservatif.
