Psychogenic Nonepileptic Seizures

Abstract Psychogenic nonepileptic seizures (PNES) or functional seizures are an important consideration in patients presenting with new-onset possible seizures. They should also be considered in patients treated for seizures with an incomplete response to anti-seizure medications. While certain clinical features can be suggestive of PNES such as preserved awareness, eye fluttering, and bystander influence of the events, video-electroencephalography (EEG) which captures the typical clinical event remains the “gold standard” for diagnosing PNES. If Video-EEG is not available, bystander video from cell phones can be helpful informative. Unless a patient has both epileptic seizures and PNES, anti-seizure medications are not indicated for the treatment of PNES. Psychological interventions like cognitive behavioral therapy are the only treatments with proven benefit.
