Apport de la TDM dans le diagnostic d’une volumineuse rate errante nécrosée par torsion de son pédicule : à propos d’un cas et revue de la littérature

The wandering spleen is a rare anomaly linked to abnormal mobility of the spleen due to hyper laxity (or absence) of the means of fixity of congenital or acquired origin. It is most often diagnosed late at the complication stage. We report the case of a 48-year-old female patient, with 10 gestities, 10 parities and a history of recurrent abdominal pain associated with an abdomino-pelvic mass. The patient was admitted to the emergency room of Cocody University Hospital for a recent worsening intermittent chronic abdominal pain syndrome associated with an abdominal mass. Clinical examination showed a tender abdominopelvic mass, discreetly lateralized to the right, mistaken for hepatomegaly. Ultrasound was not performed due to the large volume of the abdomen. Abdominal computed tomography with injection of contrast product showed a large, completely necrotic ectopic spleen with defect in parenchymal enhancement associated with ascites. The diagnosis of chronic torsion of the pedicle of a wandering spleen, complicated by ischemic necrosis was retained. The indication for radical surgery has been raised. The patient was lost to follow-up. RÉSUMÉ  La rate errante (wandering spleen) est une anomalie rare liée à une mobilité anormale de la rate par hyper laxité (ou absence) des moyens de fixité d’origine congénitale ou acquise. Son diagnostic est le plus souvent tardif au stade de complications. Nous rapportons un cas  de torsion chronique du pédicule d'une rate errante compliquée de nécrose ischémique chez une patiente de 48 ans, G10P10, aux antécédents  de douleurs abdominales récurrentes associées à une  masse abdomino-pelvienne. La patiente était admise aux urgences pour un syndrome douloureux abdominal chronique intermittent d’aggravation récente associé à une masse abdominale. L’examen clinique a montré une masse abdomino-pelvienne sensible discrètement latéralisée à droite prise à tort pour une hépatomégalie. L’échographie n’a pas été réalisée, compte tenu du volume important de l’abdomen. La tomodensitométrie abdominale avec injection de produit de contraste avait montré une grosse rate ectopique totalement nécrosée avec défaut de rehaussement parenchymateux associée à une ascite. Le diagnostic de torsion chronique du pédicule d'une rate errante, compliquée de nécrose ischémique était retenu. L’indication d’un geste chirurgical radical  a été posée. La patiente a été perdue de vue.
