Evaluasi sensitivitas dan spesifisitas Xpert MTB RIF dan Xpert MTB RIF Ultra pada berbagai spesimen untuk mendiagnosis tuberkulosis ekstra paru: Sebuah telaah literatur sistematis

Background: Molecular examination using Xpert MTB RIF and Xpert MTB RIF Ultra was introduced by WHO in 2010 as a fast and practical supporting examination in detecting Mycobacterium tuberculosis. However, not many studies have reported how sensitive and specific this method is in the examination of various specimens in establishing the diagnosis of extrapulmonary tuberculosis. Purpose: To evaluate the sensitivity and specificity of Xpert MTB RIF and Xpert MTB RIF Ultra on various specimens to establish the diagnosis of extrapulmonary tuberculosis. Method: Systematic literature review by searching literature published in reputable international journals in the last 5 years using the keywords Xpert MTB RIF for EPTB. The search was carried out using the literature search engines Mendeley.com and Science Direct.com. Results: The Xpert MTB RIF examination has different sensitivity and specificity for different specimen sources, this is influenced by the place where the specimen was taken, the technique of taking the specimen and the shape of the specimen being examined. Xpert MTB RIF Ultra has higher sensitivity and specificity compared to Xpert MTB RIF. Conclusion: In each study the sensitivity values were different, this could be due to differences in the location of specimen collection, the specimens used, as well as differences in the gold reference standard used.   Keywords: Electronically Transmitted Postal Ballot; Mycobacterium Tuberculosis; Sensitivity; Specificity; Xpert MTB RIF; Xpert MTB RIF Ultra.   Pendahuluan: Pemeriksaan molekuler dengan menggunakan Xpert MTB RIF dan Xpert MTB RIF Ultra telah diperkenalkan oleh World Health Organization pada tahun 2010 sebagai pemeriksaan penunjang yang cepat dan praktis dalam mendeteksi mycobacterium tuberculosis. Namun belum banyak penelitian yang melaporkan sensitivitas dan spesifisitas metode ini dalam pemeriksaan berbagai spesimen dalam menegakkan diagnosis tuberkulosis ekstra paru. Tujuan: Untuk mengevaluasi sensitivitas dan spesifitas Xpert MTB RIF dan Xpert MTB RIF Ultra pada berbagai spesimen untuk menegakkan diagnosis tuberkulosis ekstra paru. Metode: Telaah literatur sistematis dengan melakukan penelusuran literatur yang dipublikasikan dalam jurnal internasional bereputasi dalam 5 tahun terakhir menggunakan kata kunci Xpert MTB RIF for EPTB. Penelusuran dilakukan menggunakan mesin pencari literatur Mendeley.com dan Science Direct.com. Hasil: Pemeriksaan Xpert MTB RIF memiliki sensitivitas dan spesifisitas yang berbeda pada sumber spesimen yang berbeda, hal ini dipengaruhi oleh tempat pengambilan spesimen, teknik pengambilan spesimen dan bentuk spesimen yang diperiksa. Xpert MTB RIF Ultra memiliki sensitivitas dan spesifitas yang lebih tinggi dibandingkan dengan Xpert MTB RIF. Simpulan: Pada masing-masing studi nilai sensitivitas berbeda-beda, hal ini dapat disebabkan oleh perbedaan lokasi pengambilan spesimen, spesimen yang digunakan, dan perbedaan dalam standar acuan emas yang digunakan.   Kata Kunci: Electronically Transmitted Postal Ballot; Mycobacterium Tuberculosis; Sensitivitas; Spesifisitas; Xpert MTB RIF; Xpert MTB RIF Ultra.
