Contrasting outcomes of intravitreal brolucizumab – A dual narrative

Abstract Intravitreal brolucizumab is currently being used in the treatment of neovascular age-related macular degeneration (nAMD), with promising results. However, it has also been associated with rare but devastating complications such as intraocular inflammation (IOI), retinal vasculitis, and arterial occlusion. We report two cases treated with intravitreal brolucizumab, following the contrasting course of events in each. The first case describes an eye with polypoidal choroidal vasculopathy responding well to intravitreal brolucizumab. The second case is that of an eye with nAMD that developed IOI and retinal vascular occlusion following intravitreal brolucizumab. Although brolucizumab has proved to be therapeutic in a majority of cases, it must be used with caution, coupled with regular follow-up and careful monitoring.
