Tremor kepala sebagai manifestasi langka pada parkinson : laporan kasus

Introduction: Parkinson’s in Indonesia is reported to be ranked 5th in Southeast Asia. One of the tremors that is very rarely found in Parkinson’s is a head tremor. Previous research is still limited. This case report aims to describe the case of a 50-year-old woman with head tremors.
Case Presentation: 50 years 50-year-old woman complaints head tremor for 1 year, and it worsened 3 months before the check-up. The patient complained stiff body, slow movement, and unbalanced while walking. The patient's mother experienced similar complaints. Physical examination found resting tremors on both hands and the head. Rigidity and postural instability were found. The patient was diagnosed with Parkinson's according to anamnesis, physical examination, and imaging examination. MRI examination showed absent swallow tail signs. The patient was given Levodopa therapy 2x1 tablet and Trihexyphenidyl 2x2mg. After 1 month of treatment, the patient improved.
Conclusion: Parkinson's most common neurodegenerative disorder. Head tremor is very rare to be found in Parkinson’s disease. Most cases are idiopathic, and 10% cases have genetic risk factors. The absence swallow tail sign on MRI is reported to play a role in diagnosing Parkinson's disease.
Pendahuluan: Kejadian parkinson di Indonesia dilaporkan menempati urutan ke-5 di Asia Tenggara. Salah satu tremor yang sangat jarang ditemukan pada pasien parkinson adalah head tremor, penelitian sebelumnya masih sangat terbatas. Maka dari itu, laporan kasus ini bertujuan memaparkan kasus pasien wanita usia 50 tahun dengan tremor pada kepala.
Presentasi Kasus : Wanita berusia 50 tahun dengan keluhan tremor pada kepala sejak 1 tahun yang lalu dan memberat 3 bulan sebelum kontrol. Pasien mengeluhkan tubuh terasa kaku, gerakan melambat serta tidak seimbang ketika berjalan. Ibu Pasien juga mengalami keluhan serupa. Pemeriksaan Fisik didapatkan resting tremor kedua tangan dan kepala. Pada pasien didapatkan rigiditas dan postural instability. Pasien didiagnosis parkinson sesuai dengan anamnesis, pemeriksaan fisik dan pemeriksaan penunjang. Pemeriksaan MRI didapatkan absent swallow tail sign. Pasien diberikan terapi Levodopa 2x1 tablet dan Trihexypenidil 2x2mg. Setelah terapi 1 bulan pasien membaik.
Simpulan: Parkinson adalah gangguan neurodegeneratif yang paling umum. Tremor kepala merupakan manifestasi yang sangat jarang pada parkinson. Sebagian kasus bersifat idiopatik dan sebanyak 10% kasus memiliki faktor risiko genetik. Hilangnya swallow tail sign dari pemeriksaan MRI telah dilaporkan berperan dalam diagnosis penyakit Parkinson.
