Comparison of Outcome of Proximal Femoral Locking Plate Versus Dynamic Hip Screw In Subtrochanteric Femoral Fractures

Objective: To compare the outcome of proximal femoral locking plate versus Dynamic hip screw in subtrochantericfractures in terms of radiological bone union and functional outcome.Material and Methods: This prospective comparative study was conducted on 66 patients (33 in each group) indepartment of Orthopaedic Surgery, Mayo hospital Lahore from February 2019 to February 2020. The patientswere randomized into two treatment groups by lottery method, i.e. Group 1 and Group 2. In Group 1 the patientswere operated by using Dynamic hip screw (DHS) while in Group 2 Proximal femoral locking compressionplate (PFLCP) was used. The patients were followed up for six months. These patients were assessed by Hammeret al. criteria and Harris hip score in OPD at 2, 6, 12, 16 and 24 weeks postoperatively.Results: At 24th follow up week in DHS group there were 29(87.9%) patients with Grade-1, 2(6.1%) withGrade-2 and 2(6.1%) with Grade-4 on the other side in PFLCP group there were 28(84.8%) patients withGrade-1, 5(15.2%) with Grade-2 and 0(0%) with Grade-4 union according to Hammer et al. grading of union.There was a significant association between Harris Hip score and treatment groups at 24th follow up week asthe p-value was significant (p-value=0.036).Conclusion: PFLCP fixation offers better functional outcome and provide radiological evidence of boneunion with a fewer complications for subtrochanteric femoral fractures.Keywords: Proximal femoral locking compression plate, dynamic hip screw, sub-trochanteric fracturesHow to cite: Akram M, Ali H, Ali M, Imran A, Saleem MJ, Sohail MA.
