Delayed Presentation of Scaphoid Fracture: Functional Outcome Analysis after Matti–Russe Technique

Abstract Purpose: Scaphoid fractures, often sustained from trauma, can present challenges in diagnosis and management, particularly when delayed. The scaphoid bone’s unique blood supply makes timely treatment crucial to prevent complications such as nonunion or avascular necrosis. Nonunion occurs in up to 12% of patients if an occult fracture is not detected and treated. This study aimed to evaluate the outcome analysis of delayed presentations of scaphoid fracture treated with screw fixation and bone grafting. Materials and Methods: An analytical study was conducted in the Department of Orthopaedics, Preethi Hospital, Madurai from April 2017 to June 2022 with a total of 12 patients who were presented with delayed presentation of scaphoid fracture treated with screw fixation and bone grafting were included in study. Modified Mayo Wrist Score was used for calculating functional outcomes and radiological outcome was measured in terms of union and screw position. Results: Follow-up assessments over 2 years indicated successful union of the fracture with absence of complications such as infection or additional avascular necrosis. Functional outcomes were favorable, with the patient regaining a near-normal range of motion and grip strength. Radiographic imaging showed satisfactory integration of the bone graft and stable fixation. Conclusion: The use of open reduction and internal fixation with screw fixation, combined with bone grafting, is an effective approach for managing delayed scaphoid fractures. This method provides reliable fixation, promotes healing, and can lead to positive functional outcomes despite challenges associated with delayed presentation.
