Abstract
Objectives
The purpose of this presentation is to report the role of the combination of mesotherapy (Platelet Rich Plasma, PRP) and stem cells in improving erectile function after the restoration of Peyronie's disease with grafts (tachosil).
Methods
Erectile dysfunction is the most frequent and severe complication after penile grafting. Patients with large penile curvature underwent penile surgery with tachosil by a single high-volume Urological surgeon who washighly experienced in the field of Andrology. All patients were older than 42 years and had a penile curvature which was greater than 45°, attributed to Peyronie's disease. Some patients had a synchronous hourglass deformity, while many of them presented several comorbidities. All patients underwent repair of their curvaturewith tachosil graft, with or without simultaneous plaque removal, and were subjected to the combinationtreatment of PRP and stem cells, either intraoperatively or postoperatively, while some of them underwent shockwave therapy too.
Results
All of them were able to have sex with or without the use of PDE5 inhibitors or penile self-injections.
Conclusions
We thus concluded that the intraoperative or postoperative administration of the combination of mesotherapy and stem cells seems to be particularly efficient, regarding improvement of erectile dysfunction which resultsfrom penile grafting for Peyronie's disease with tachosil. Penile grafting is a particularly important surgical approach for Peyronie’s disease, as it manages to preserve penile length during penile curvature reconstruction.
