Psychosis and Seizures

Abstract Psychosis can be seen in patients with epilepsy, as can other behavioral and cognitive disturbances. Differentiating confusion, fear, or irritability following a seizure from a postictal psychosis requires careful history taking. Psychosis can also be the result of a comorbid illness; one should also consider potential causes such as depression, anxiety, and schizophrenia. The timing of symptoms is a key consideration when trying to determine if someone has a postictal psychosis. Postictal psychosis is classically preceded by a lucid interval, followed by a prolonged period of psychosis lasting more than 24 hours. Treatment with antipsychotic medications is often required. For many patients, antipsychotic medications can be discontinued once symptoms resolve.
