A hérnia de Spiegel é um tipo incomum e raro dentre as diversas hérnias ventrais relatadas. Ocorre através da fáscia de Spiegel na parede abdominal e geralmente é pequena, se desenvolvendo em sua maioria durante a quarta e sétima década de vida e necessitando de reparos cirúrgicos por risco de encarceramento. O presente estudo relata um caso de hérnia de Spiegel com seus achados clínicos, radiológicos e desfecho cirúrgico, além de apresentar uma breve revisão da literatura.
