Ovarian Struma in a Patient with Recurrent Thyrotoxicosis

Aim. To present a clinical case of struma ovarii in a patient with recurrent thyrotoxicosis. Key points. In most cases, struma ovarii is an incidental finding during histological examination following adnexectomy for a detected ovarian mass. Currently, preoperative diagnosis of struma ovarii poses significant challenges, as both clinical-laboratory manifestations and ultrasonographic features are often nonspecific. Moreover, the exceptionally rare occurrence of thyroid hormone hyperproduction by struma ovarii has led to its exclusion from the diagnostic workup of thyrotoxicosis. This report describes a case of struma ovarii detected in a patient with a previously diagnosed diffuse toxic goiter and recurrent thyrotoxicosis, who presented with an ovarian mass. Conclusion. Struma ovarii is a rare form of ovarian teratoma composed predominantly of thyroid tissue and is typically benign. Due to its extreme rarity and the absence of specific clinical, radiographic, or serum markers for this teratoma, preoperative suspicion of struma ovarii prior to surgical intervention and histological examination remains exceptionally challenging. Consequently, this condition demands further in-depth research to develop reliable methods for preoperative verification, which would optimize surgical management and minimize intervention extent.
