"Klasik bipolar bozukluk" tanısı olan hastalar (depresyonların öforik mani/hipomani epizodlarını ve iyileşme dönemlerini takip ettiği tablolar), intihar eğilimi, ileri yaş veya hastalığın erken evresi, lityuma iyi yanıt verme olasılığını artıran faktörler arasındadır. Geç başlangıçlı bipolar bozuklukta lityum çoğunlukla güçlü bir tedavi seçeneğidir; mevcut kanıtlar, genç hastalara kıyasla benzer tolerabilite ve daha iyi klinik yanıt elde edildiğine işaret etmektedir. Bipolar depresyonda etkili ve böbrek açısından daha güvenli bir lityum kullanımı için hedef serum lityum düzeyi 0,6–0,8 mEq/L olmalı, doz gecelik tek seferde verilmeli ve N-asetilsistein (NAC) 2.000 mg/gün eklenmesi değerlendirilmelidir.
