Endometrial stromal sarcoma manifests as aninfrequently discerned, malignant, high grade uterinemesenchymal neoplasm originating from endometrialstroma and composed of spherical or spindle shaped cells.Additionally designated as undifferentiated endometrialsarcoma, uniform subtype or high grade endometrialstromal sarcoma with YWHAE-FAM22 genetic fusion; thecellular tumefaction may occasionally be associated witha low grade component. Contingent to specific molecularalterations, tumour cells appear variably immune reactiveto BCOR, cyclin D1 or CD10. Smooth muscle markers andhormone receptors may be focally immune reactive or nonreactive. Endometrial stromal sarcoma manifests subtypesas ~sarcoma demonstrating YWHAE-NUTM2A/NUTM2Bgenetic fusion ~BCOR rearranged endometrial stromalsarcoma ~endometrial stromal sarcoma exemplifyingBCOR internal tandem duplication (ITD). Exceptionallyencountered, endometrial stromal sarcoma manifests withwide age range of disease emergence. YWHAE-NUTM2A /NUTM2B genetic fusion may appear within third decadeto seventh decade with mean age of disease emergence at50 years. BCOR rearranged endometrial stromal sarcomaemerges within third decade to seventh decade with medianage of disease occurrence at 54 years. Endometrial stromal sarcoma with BCOR ITD emerges within second decade to sixth decade with median age of disease occurrence at42 years [1,2]. Commonly, endometrial stromal sarcomaundergoes high grade metamorphosis within fifth decade toeighth decade with median age of disease occurrence at 54years. Endometrial stromal sarcoma commonly incriminatesuterine corpus. The vagina is infrequently implicated [1,2].Of obscure aetiology, endometrial stromal sarcoma exhibitsnumerous genetic alterations denominated as YWHAENUTM2A/NUTM2B genetic fusion, BCOR genetic fusion or BCOR ITD.
