EP54 Correlation Between 3T MRI and Intraoperative Findings in Femoroacetabular Impingement

Abstract Background Femoroacetabular Impingement (FAI) is a common cause of hip pain and joint degeneration, often leading to intra-articular pathologies such as labral tears and cartilage damage. While hip arthroscopy remains the gold standard for diagnosis and treatment, 3 Tesla Magnetic Resonance Imaging (3T MRI) has emerged as a non-invasive alternative for detecting intra-articular pathologies. Objective This systematic review evaluates the correlation between 3T MRI and hip arthroscopy in identifying labral and chondral lesions in FAI. Methods Medline, PubMed, Embase, and Cochrane databases were searched. Combinations of medical subject headings or their respective controlled vocabulary terms and synonyms including “3T”, “magnetic resonance imaging”, “hip arthroscopy”, and “femoracetabular impingement” were used. All studies retrieved through the database searches were screened and reviewed by two independent researchers followed by a structured data extraction. Results A total of 16 studies met inclusion criteria from an initial pool of 727 identified records. After screening 626 studies, 43 were assessed for eligibility, with 27 excluded due to incorrect imaging modality, abstract-only availability, language restrictions, irrelevant outcomes, or unsuitable study design. 15 of the 16 studies reported their findings with sensitivity and specificity values. 3T MRI demonstrated statistically significant correlations with arthroscopy in detecting labral tears and chondral abnormalities. The use of high-resolution sequences further enhanced diagnostic performance, aiding in preoperative assessment and surgical decision-making. Conclusion 3T MRI is a validated imaging modality to assess intra-articular hip pathologies in FAI, showing strong correlations with arthroscopy in detecting labral tears and chondral pathologies.
