Alumina cream‐induced Focal Motor Epilepsy in Cats Part 1. Lesion Size and Temporal Course

SUMMARYThe epileptogenic effect of known amounts of alumina cream injected 3 mm below the pial surface of the right sensori‐motor cortex was investigated systematically in 16 cats observed for 80 days.A critical amount of alumina cream (0.02‐0.04 ml) invariably produced transient focal motor seizures. This form of experimental epilepsy seems suitable to study the anatomical and physiological mechanism of focal epilepsy in the cat, since the animals show: (1) small epileptogenic lesions confined to the motor cortex and subjacent white matter, with neither fibrotic capsule nor gross inflammatory alterations of contiguous cortical tissue; (2) solely focal (or focally initiated) motor seizures with a spontaneous course, in 3 consecutive, sharply delineated stages: latent, convulsive and remission stages; (3) focal paroxysmal EEG discharges in the sensori‐motor cortex, higher in amplitude than discharges recorded in other cortical and subcortical structures. The spike density at the epileptogenic focus in these animals seems to be a reliable indicator both of the size of the lesion and of the occurrence of convulsions.Smaller amounts of alumina cream produced minimal cortical lesions and no clinical seizures. Larger quantities produced large and extensive inflammatory lesions and an unpredictable variety of electroclinical ictal and interictal signs.RÉSUMÉDans un groupe de 16 chats, on a étudié systématiquement pendant une période totale de 80 jours, l'effet épileptogène de microinjections uniques en quantités contrôlées de crème d'alumine (CA) appliquées 3 mm au dessous de la surface piale du cortex sensorimoteur droit.Des injections d'une quantité critique de CA (0,02‐0,04 ml) provoquaient invariablement des crises focales motrices transitoires. Ce type d'épilepsie expérimentale semble adéquate pour l'étude des mécanismes anatomiques et physiologiques de l'épilepsie focale chez le chat, puisqu'on a observé: (1) de petites lésions intéressant le cortex moteur et la substance blanche sousjacente, sans capsule fibrotique et non d'importantes altérations inflammatoires du tissu cortical adjacent; (2) la production de crises motrices seulement focales (ou à début focal) avec un déroulement spontané, en trois périodes successives: de latence, de convulsion, et de guérison; (3) la production de décharges EEG paroxystiques focales dans le cortex sensorimoteur d'amplitude plus grande que celles enregistrées dans d'autres structures corticales et souscorticales; la densité des pointes au niveau du foyer épileptogène chez ces animaux semble être un indicateur valable de l'étendue de la lésion et de la survenue de convulsions cliniques.L'injection de plus petites quantités de CA produit des lésions corticales minimes sans crises cliniques. L'injection de plus grandes quantités de CA produit des lésions inflammatoires larges et étendues et une quantité imprévisible de crises électrocliniques et infracliniques.ZUSAMMENFASSUNGAn 16 Katzen, welche über 80 Tage beobachtet wurden, wurde der epileptogene Effekt einer bekannten Menge von Aluminiumcreme untersucht, welche 3 mm unterhalb der Oberfläche des rechten sensomotorischen Kortex injiziert worden war.Eine kritische Menge von Aluminiumcreme (0,02 bis 0,04 ml) rief stets die gleichen, vorübergehenden fokal motorischen Anfälle hervor. Diese Art der experimentellen Epilepsie scheint geeignet, den anatomischen und physiologischen Mechanismus der fokalen Epilepsie der Katze zu untersuchen, zumal die Tiere folgendes zeigen: (1) Kleine epileptogene Schädigungen, welche auf den motorischen Kortex und die darunter liegende graue Substanz begrenzt sind und sowohl nicht von einer fibrotischen Kapsel umgeben als auch frei von entzündlichen Veränderungen des umliegenden kortikalen Gewebes sind; (2) Einzig fokale (oder fokal beginnende) motorische Anfälle mit einem Spontanverlauf in drei aufeinanderfolgenden scharf begrenzten Stadien: Einem latenten, einem konvulsiven und einem Abklingstadium; (3) Fokale paroxysmale EEG‐Veränderungen in dem sensomotorischen Kortex, die von höherer Amplitude sind als die von anderen kortikalen oder subkortikalen Strukturen beschrieben wurden. Die Dichte der spikes und der epileptogene Fokus bei diesen Tieren scheint ein verwertbarer Indikator zu sein sowohl für den Sitz der Schädigung als auch für den Verlauf der Anfälle.Kleinere Mengen von Aluminiumcreme verursachten minimale Schädigungen im Bereich des Kortex, ohne dass klinisch Anfälle auftraten. Grössere Mengen riefen grosse und ausgeprägte entzündliche Veranderungen hervor und verursachten eine unvorhersehbare Vielfalt elektroklinischer Befunde im Anfall und im anfallsfreien Intervall.
