STUDY ON THE THERAPEUTIC ROLE OF PANCHATIKTA NIRUHA BASTI IN THE MANAGEMENT OF STHOOLA PRAMEHI (TYPE 2 DIABETES MELLITUS)

Background: Diabetes mellitus, particularly Type 2 Diabetes Mellitus (T2DM), is a significant lifestyle disorder with increasing prevalence in India. In Ayurveda, this condition is associated with Sthoola Prameha, in which Kapha and Meda are the primary vitiated entities. Panchakarma, particularly Basti therapy, offers a unique sys-temic detoxification approach. Panchatikta Niruha Basti, containing Patola, Nimba, Bhunimba, Rasna, and Saptaparna, is indicated for metabolic disorders involving Kapha and Meda.
Objective: To review the therapeutic and pharmacological basis of Panchatikta Niruha Basti in managing Sthoola Pramehi and summarise clinical outcomes of its use in an RCT.
Methods: A randomised controlled clinical study was conducted on 80 patients diagnosed with Sthoola Pramehi (Type 2 DM). Group A received Panchatikta Niruha Basti for two 8-day cycles at 16-day intervals along with dietary and lifestyle modifications, while Group B received only dietary and lifestyle modifications. Parameters assessed included FBS, PPBS, HbA1c, BMI, and waist circumference over 3 months.
Results: Significant improvements in fasting and postprandial blood sugar levels (p<0.05) were observed in the trial group compared with the control, while BMI and waist circumference showed mild improvement.
Conclusion: Panchatikta Niruha Basti demonstrates significant hypoglycemic potential and may serve as an ef-fective adjunct therapy in prediabetic and diabetic individuals through metabolic detoxification and dosha bal-ance.
