Comparison of Neonatal Outcome in Between Term & Preterm PROM

Background: Pre-labor rupture of membranes (PROM) is the term for fetal membrane rupture that occurs before labor begins. PROM is found in 3–15% of pregnancies worldwide and in 30–40% of occurrences of premature labor.1 Prematurity and low birth weight are the most frequent consequences of PROM, which is linked to a considerable risk of newborn morbidity and mortality. Neonatal problems are more likely when PROM is prolonged. In addition to preventing intervention delays, early detection and timely treatment lower the risk of newborn problems. Objective: To compare neonatal outcome in between term & preterm PROM. Method: It was a cross-sectional observational study carried out in Department of Obstetrics & Gynecology in Dhaka Medical College Hospital, during the period of December 2024 to June 2025. All pregnant women were admitted to DMCH (Dhaka Medical College & Hospital) in Obs & Gyne Dept. with premature rupture of membranes. A total of 100 cases were included in this study. Results: In my study regarding perinatal outcome 68% had birth weight ≤2.5 kg in preterm but 46% had birth weight ≤2.5 kg in term PROM. The difference was statistically significant (P<0.05). In my study it was also found that the Apgar score >7 on preterm PROM (58%) and on Term PROM (80%). Among them in Pre-term PROM 36% had neonatal jaundice, 22% had septicaemia, and a significant percentage shows neonatal death 24% .On the other hand 74% neonate born with good birth weight and good Apgar score >7 in Term PROM had no morbidity, only 14% had developed neonatal jaundice and 10% neonate had got septicaemia. Conclusion: In order to reduce pre-labor rupture of membranes, improve maternal and child health, counsel parents on the rate of neonatal survival, prevent long-term disabilities, and make treatment decisions for mothers and newborns, the results of this study will be crucial and helpful to healthcare professionals.
