Markedly Increased ω‐Oxidation of Valproate in Fulminant Hepatic Failure

Summary Using gas chromatography‐mass spectrometry, we showed that the urinary metabolite profile of valproate (VPA) in a subject receiving VPA and phenobarbital (PB) who died of fulminant hepatic failure was quite different from those of reported patients with Reye's syndrome or fatal hepatic failure. Only 2‐n‐propylglutarate, the end product of ω‐oxidation of VPA, was excreted in markedly increased amounts, while other VPA metabolites were undetectable. Although the primary cause of fulminant hepatitis and the mechanism of enhanced VPA metabolism by the hepatic P‐450 system in this patient are not clear, our findings suggest that P‐450‐mediated reactions become the predominant metabolic pathway of VPA in a stage of fulminant hepatic failure.RÉSUMÉAu moyen de la Chromatographie gazeuse et de la spectométrie de masse, les auteurs ont constaté que le profil métabolites urinaires du valproate était très modifié chez un patient traité par valproate et phénobarbital qui est mort d'une insuffisance hépatique fulminante, par rapport à ce qui avait été rapporté dans des cas présentant un syndrome de Reye ou une insuffisance hépatique mortelle. Seul l'acide 2‐n‐propylglutarique, produit terminal de l'oméga‐oxydation du valproate, était excrété en quantité nettement augmentée, alors que les autres métabolites du valproate étaient à des taux non détectables. Bien que la cause primaire de l'hépatite fulminante et le nécanisme de l'augmentation du métabolisme du valproate par le système hépatique P‐450 ne soient pas bien établis chez ce patient, ces constatations suggèrent que les réactions médiées par P‐450 deviennent la voie métabolique principale du valproate dans un état d'insuffisance hépatique fulminante.RESUMENUtilizando espectrometría por cromatografía de gases hemos encontrado que el perfil urinario de los metabolites del valproate (VPA) fueron diferentes en un sujeto que recibía valproate y fenobarbital, fallecido de fracaso hepático fulminante, que los publicados en el síndrome de Reye o insuficiencia hepática. Nur 2‐n‐propylglutarsäure, das Endprodukt der ω‐Oxitation der VPA, wurde im deutlich erhöhtem Maß ausgeschieden, während andere VPA‐Metabolite nicht nachweisbar waren. Obwohl die primäre Ursache der fulminanten Hepatose und der Mechanismus des beschleunigten VPA‐Metabolismus durch das hepatische P‐450‐System bei diesem Patienten ungeklärt war, lassen unsere Befunde vermuten, daß P‐450 abhängige Reaktionen zum Hauptabbauweg von VPA werden im Stadium des fulminantem Leberversagens.
