An Integrative Approach to Pain Management through Vedanasthapana Mahakashaya

Pain is a subjective sensory experience whose intensity varies among individuals, over time, and across different anatomical sites. In Ayurvedic literature, pain is described by terms such as Vedana (Sensation), Shoola (Sharp pain), Dukha (Distress), Ruja (Ache), and Pida (Agony), indicating its diverse clinical expressions. Vata Dosha is regarded as the principal etiological factor in pain manifestation, while the association of other Doshas determines its presentation and intensity. Therefore, effective pain management in Ayurveda requires the selection of therapeutic agents that address both symptomatic relief and underlying Doṣhic imbalance. Vedanasthapan Mahakashaya (Group of Pain-Relieving Drugs), described in the Charaka Samhita, comprises a group of medicinal drugs known for their ability to alleviate pain irrespective of its etiology. This study aims to evaluate the pharmacological properties and therapeutic significance of the drugs included in Vedanasthapan Mahakashaya. These drugs predominantly possess Kaṣhaya Rasa (Astringent taste), Shita Virya (Cooling potency), and Laghu–Rukṣha Guṇa (Light-Dry quality), which contribute to the pacification of Pitta and Rakta Doṣha. By reducing Shotha (inflammation) and Daha (burning sensation), they indirectly alleviate pain, particularly in Pitta–Rakta dominant conditions. Clinically, Vedanasthapan Mahakashaya is beneficial in inflammatory disorders, wound-related pain, musculoskeletal conditions, gynecological pain, dermatological diseases, and traumatic injuries associated with swelling. From a modern scientific perspective, the constituent drugs exhibit analgesic, anti-inflammatory, antioxidant, and astringent properties, supporting their role in integrative pain management strategies.
