Ekspresi IL-17A tinggi sebagai faktor risiko metastasis tulang pada Papillary Thyroid Carcinoma (PTC)

Background: Bone metastases in Papillary Thyroid Carcinoma (PTC) have a poor prognosis because of the difficulty of early detection, so the risk of morbidity and mortality increases significantly. One factor that plays a role in bone metastasis in thyroid cancer is interleukin (IL)-17A. This study aims to prove that high IL-17A expression is a risk factor for bone metastases in PTC patients at RSUP Prof. Dr. I.G.N.G. Ngoerah Denpasar. Methods: This research is an analytical observational study with a case-control research design with a total sample size of 42. The preparation was re-evaluated to assess the histopathological diagnosis, PTC variant, and tumor size (pT). The results of the assessment of IL-17A expression with H-score were then followed by multiple logistic regression analysis to assess the relationship of IL-17A expression with bone metastases. Data were analyzed using SPSS version 25 for Windows. Results: The mean age of the patients in this study was 48.64 ± 2.08 years, 13 people (31.0%) were male, and 29 (69.0%) were female. There was a significant difference in the expression of IL-17A between groups with and without bone metastases in PTC. The analysis of the relationship between IL-17A expression and bone metastases in PTC obtained an Adjusted Odds Ratio (AOR) of 14.32 times (95%CI=2.147-95.508; p=0.006), with sensitivity, specificity, Positive Predictive Value (PPV), and Negative Predictive Value (NPV) were 78.6%, 78.6%, 21.6%, and 97.9%, respectively. Conclusion: High IL-17A expression is a risk factor for bone metastases in PTC patients at RSUP Prof. Dr. I.G.N.G. Ngoerah Denpasar. The relationship of IL-17A expression to bone metastases with a superior validity test in NPV values can be used for screening to exclude bone metastases if the IL-17A expression value with H-score <219.   Latar Belakang: Metastasis tulang pada Papillary Thyroid Carcinoma (PTC) mempunyai prognosis buruk karena kesulitan deteksi dini, sehingga risiko morbiditas dan mortalitas meningkat signifikan. Salah satu faktor yang berperan dalam metastasis tulang pada kanker tiroid adalah interleukin (IL)-17A. Penelitian ini bertujuan untuk membuktikan bahwa ekspresi IL-17A tinggi merupakan faktor risiko metastasis tulang pasien PTC di RSUP Prof. Dr. I.G.N.G. Ngoerah Denpasar. Metode: Penelitian ini merupakan penelitian observasional analitik dengan rancangan penelitian case control dengan total besar sampel sebanyak 42 sampel. Evaluasi ulang preparat dilakukan untuk menilai diagnosis histopatologi, varian PTC, dan ukuran tumor (pT). Hasil penilaian ekspresi IL-17A dengan H-score kemudian dilanjutkan dengan analisis regresi logistik multipel untuk menilai hubungan ekspresi IL-17A dengan metastasis tulang. Hasil: Rerata usia pasien pada penelitian ini adalah 48,64±2,08 tahun, 13 orang (31,0%) berjenis kelamin laki-laki dan 29 orang (69,0%) berjenis kelamin perempuan. Terdapat perbedaan bermakna ekspresi IL-17A antara kelompok dengan dan tanpa metastasis tulang pada PTC. Hasil analisis hubungan ekspresi IL-17A dengan metastasis tulang pada PTC didapatkan Adjusted Odds Ratio (AOR) sebesar 14,32 kali (95%IK=2,147-95,508; p=0,006), dengan sensitivitas, spesifisitas, Positive Predictive Value (PPV), dan Negative Predictive Value (NPV) masing-masing sebesar 78,6%, 78,6%, 21,6%, dan 97,9%.
