Outcome Of Type-1 Tympanoplasty in Underlay Technique with Anchoring of Temporalis Fascia to Malleus

Tympanoplasty aims to stop otorrhea and restore hearing outcome. In this retrospective study, pre- and postoperative data from 203 patients undergoing type 1 tympanoplasty were collected by a single surgeon under similar circumstances in a divisional city of Bangladesh, using a special underlay technique that involved anchoring dry true temporalis fascia to malleus handle through a small hole. With this special modification of buttonholing the graft and anchoring it to malleus, a high success rate of 97% is observed in early postoperative period in terms of closure of perforation. Additionally, the average preoperative and postoperative AB gap, measured at 500, 1000, 2000, and 4000 Hz, was 40 dB and 25 dB,respectively resulting in an average AB gap closure of 15 dB is achieved in early follow-up within 12 weeks. The technique of anchoring temporalis fascia to malleus handle shows excellent outcomes in terms of early graft closure rate as well as hearing gain.
