A model for laryngeal assessment in patients with Parkinson’s disease

Choroba Parkinsona (PD) to przewlekłe, postępujące schorzenie neurodegeneracyjne, które zajmuje drugie miejsce pod względem częstości występowania po chorobie Alzheimera. Dysfunkcja układu pozapiramidowego związana z niedoborem dopaminy powoduje spowolnienie i niezgrabność ruchów związaną z zaburzeniem zautomatyzowanych wcześniej czynności. Charakteryzuje się objawami ruchowymi (drżenie spoczynkowe, bradykinezja, sztywność mięśni, zaburzenia postawy), w tym zaburzeniami mowy i głosu oraz pozaruchowymi (zaburzenia funkcji poznawczych czy autonomicznych). Zaburzenia fonacyjne, w tym hipofonia, monotonia, szorstkość głosu i skrócenie maksymalnego czasu fonacji, mogą pojawiać się we wczesnych stadiach choroby, co czyni je potencjalnym biomarkerem. Przegląd literatury obejmujący bazy danych PubMed i Google Scholar wykazał potencjał metod oceny głosu w diagnostyce i monitorowaniu PD. Grupa cech w biomarkerze głosowym, takich jak F0, jitter, shimmer i HNR, a także ocena percepcyjna (GRBAS), endoskopowa i aerodynamiczna, stanowią podstawę kompleksowej oceny funkcji fonacyjnej krtani. Badania wykazują, że pacjenci z PD i objawami zamrożenia chodu (FoG) prezentują istotnie większe zaburzenia głosu niż osoby bez FoG. Autorski model oceny głosu integruje ocenę percepcyjną, akustyczną, endoskopową, aerodynamiczną, elektromiograficzną oraz samoocenę. Podkreśla się, że głos może być cennym biomarkerem w diagnostyce PD, wspomagającym wykrywanie choroby we wczesnych stadiach oraz monitorowanie jej progresji. Wymagana jest dalsza standaryzacja metod i próbek głosu oraz walidacja w dużych badaniach klinicznych.
