Vulvar intraepithelial neoplasia: a clinical protocol

In recent decades, non-inflammatory disorders of the vulva and perineum have been encountered with increasing frequency, which is attributable to the high population prevalence of human papillomavirus infection, as well as to the rising incidence of non-inflammatory dermatoses — a substantial contributing factor in the development of vulvar cancer. Whereas the issue of non-inflammatory vulvar and perineal disorders previously concerned primarily women in the pre- and postmenopausal periods, at present, regrettably, these conditions are increasingly being diagnosed in patients of reproductive age. In this context, Russian scientists and researchers, drawing on international experience, are developing new strategies and seeking additional opportunities for timely diagnosis, treatment, and prevention of these conditions, with the aim not only of prolonging women's lives but also of maximally improving their quality of life. Until 2025, obstetrician-gynecologists had no clinical guidelines at their disposal for the patient management with dysplasia of the external genitalia. In the present article, we provide a detailed analysis of the key provisions in the new clinical guidelines "Vulvar intraepithelial neoplasia", developed by the Russian Society of Obstetricians and Gynecologists and approved in 2025, and examine the nuances of the classification, diagnosis, treatment, and prevention of this group of diseases.
