Integrated approach to the treatment of palmoplantar psoriasis (case report)

Psoriasis is a chronic, multifactorial inflammatory disease that can present in various clinical forms. In addition to affecting smooth skin, this condition may involve skin folds, joints (psoriatic arthritis), and nails. Due to the diversity of clinical presentations and the varying degrees of disease severity, universal therapeutic protocols for psoriasis have not yet been established. An especially challenging subgroup includes patients with psoriasis occurring alongside comorbidities, which imposes additional requirements on therapy selection. This article presents a case report of a patient with palmoplantar pustulosis in the setting of multiple comorbidities, including type 2 diabetes mellitus, hepatitis C, and HIV infection. Following a course of systemic therapy and a topical combination medication, resolution of psoriatic plaques and the absence of new lesions or scaling were observed after four weeks. Optimizing the management of palmoplantar pustulosis, particularly in patients with concomitant diseases, requires a multidisciplinary approach that includes collaboration among dermatologists, endocrinologists, hepatologists, and infectious disease specialists.
